Cocaine Nose: Nasal Damage From Snorting Cocaine

Snorting cocaine damages the nose. That’s not a dramatic claim – it’s a straightforward consequence of how the drug works. Cocaine is a vasoconstrictor. It tightens blood vessels, which cuts oxygen supply to the tissue it contacts. Do that repeatedly, and tissue starts to break down.

The term “cocaine nose” covers the range of nasal damage that develops from regular cocaine use, from minor irritation to structural collapse. Here’s what that damage actually looks like, why it happens, and whether it can heal.

What Is Cocaine Nose?

Cocaine nose refers to nasal injury caused by snorting powdered cocaine. The drug enters the body through the nasal passage, where it comes into direct contact with the mucous membranes, cartilage, and blood vessels that line the inside of the nose.

Because cocaine restricts blood flow wherever it’s applied, repeated use starves nasal tissue of the oxygen it needs to stay healthy. Over time, that oxygen deprivation causes tissue to die. The damage is cumulative – it doesn’t reverse between uses the way a cold does.

Types of Nasal Damage From Cocaine

Nosebleeds

Frequent nosebleeds are usually the first sign. Blood vessels in the nasal lining rupture more easily when they’re chronically constricted and irritated. Someone using cocaine regularly may notice nosebleeds that start without warning or don’t stop as quickly as they used to.

Deviated Septum

The septum is the wall of cartilage dividing the two nostrils. Repeated cocaine use inflames and weakens it. A deviated septum shifts to one side, causing difficulty breathing through one nostril, chronic congestion, and sometimes a visible asymmetry in the nose.

Septal Perforation

If the tissue damage goes further, a hole can form in the septum. This is called a septal perforation. It develops when the cartilage between the nostrils loses its blood supply entirely and the tissue dies. People with perforations sometimes describe a whistling sound when they breathe, or they notice air passing through in a way that feels different.

Saddle Nose Deformity

In severe cases, the structural collapse extends to the nasal bridge. When the internal support of the nose is compromised enough, the bridge can cave inward, creating what’s called a saddle nose – named for the dip it produces in the nose’s profile. This is not reversible without surgery.

Sinus Infections

Chronic cocaine use creates an environment where sinus infections take hold and linger. The inflamed, damaged tissue can’t filter air or fight off bacteria the way healthy tissue can. Frequent sinus pressure, postnasal drip, and infections that don’t fully clear are common.

Palatal Perforation

In cases of very heavy, prolonged use, damage can extend past the septum to the palate – the roof of the mouth. A hole in the palate causes problems with speech, eating, and swallowing. It’s rarer than other forms of cocaine nose, but it does occur.

Why Does This Damage Happen?

The primary mechanism is vasoconstriction. Cocaine blocks the reuptake of norepinephrine, which keeps blood vessels tightly contracted for as long as the drug is active. Repeated cycles of constriction and release cause the vessel walls to weaken and the surrounding tissue to deteriorate.

Cocaine also typically contains cutting agents – levamisole, phenacetin, and other substances added to increase weight or potency. These additives cause their own tissue irritation on top of the damage cocaine itself creates.

Can Cocaine Nose Heal?

It depends on how far the damage has progressed.

Early-stage damage – inflammation, frequent nosebleeds, minor congestion – can improve significantly with abstinence. Once cocaine use stops, blood vessels begin to function normally again, and irritated tissue can recover. People who stop early often see meaningful improvement within weeks to months.

Structural damage is a different story. A septal perforation, saddle nose, or palatal perforation won’t close on its own. These injuries require surgical repair, and even surgery has limitations depending on how much tissue remains viable.

The most important variable is when use stops. The longer the damage continues, the less of it is reversible.
